Conduct initial assessments in the community.
Train and assess workers' competencies and allocate support workers to patients.
Collaborate with the wider multidisciplinary team (MDT).
Attend meetings and make referrals.
Hold team meetings.
Manage a team of non-clinical case managers.

The ideal Community Nurse Case Manager will:
Be an RGN or RSCN Qualified Nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management.
Have experience in tracheostomy and complex care.
Be a car driver, flexible to travel around South Yorkshire (Sheffield, Rotherham)
Have experience caring for both adults and children.
